Pursuant to 35 ILCS 200/18-50.2, the City of DeKalb (the “City”) is required to make a good-faith effort to collect and electronically publish data from all vendors and subcontractors doing business with the City as to whether the vendor or subcontractor: (1) is a minority-owned, women-owned, or veteran-owned business, as defined in the Business Enterprise for Minorities, Women, and Persons with Disabilities Act (30 ILCS 575/1, et seq.); and (2) holds any certifications for those categories or is self-certifying and a small business under federal Small Business Administration standards.
Vendors and subcontractors doing business with the City should complete the Vendor Information Reporting form.
